o    I’d be shocked if you said no!

•    Have you ever had difficulty communicating with your doctor?
o    Again, shocked, but pleased if you said no.

•    Do you know what to do when a loved one is hospitalized?
o    I know you’d drop everything and get to them, but then what?

•    What if you required hospitalization?
o    Hopefully you have a person who will drop everything for you.

•    Do you know what the following terminology means?
o    ACA (Affordable Care Act)
o    ACO (Accountable Care Organization)
o    PCMH (Patient Centered Medical Home)
